  State FFA Officer Request Form

      

This State FFA Officer Request Form is to be used to request a State FFA Officer to visit your chapter, school, special event, banquet or other activity.  Advisors are to complete the form below and when complete just click send.  Your form will arrive at my office and will be processed.  We will do our best to schedule the officer(s) you requested, but it is subject to change. You may be asked to provide directions to the officer(s) once your request is submitted. You will be asked to submit a State FFA Officer Review Form after the visit or you can email your comments to the email address below.  If there are questions regarding the officer request form please contact the State FFA Coordinator at ctffa@earthlink.net or 860-377-8290.
